Attached please find a fully executed Amendment to Operating Agreement of T3 Hospitality, LLC,
which is the manager of the applicant, Gaku Ramen II, LLC.
As you will see, the Amendment explains that Kelley Jones has been removed from the company,
leaving Joseph Vicari as the sole member. When Mr. Jones’ satisfactory fingerprint background
results have been received by the BLA from IdentoGo, Mr. Jones will automatically be added back to
the company. If you have any questions, please feel free to reach out.
