Signature of mdividual responsible for system HHS Approval City of Boulder Human Relations Commission Interpretation System Rental Contract Attachment A Philosophy: The use of the HRC interpretation system by community groups and governmental agencies is an extension of the nussion of the HRC to facilitate commumcat~on between and among diverse commumties withm the crty of Boulder. The sys[em is available to groups withm the city of Boulder under the followmg circumstances• a. The group rentmg the system must demonstrate that the usage will further th~s goa] primarily m servmg low income people and m providing human services and education. b The event must take place within the city limits of Boulder. a The equipment cannot leave the city l~mits. d The renter (person signing the contract) must reside in Boulder and must be able to show photo identification verifymg this e. The event must be free and open to the public. System Description: The system is a portable wireless communication system. It uses a voice-silencmg microphone which allows for discrete simultaneous translation through a portable pocket sized transmitter to as many as SO listeners who are equipped with personal receivers and ear bud headphones. The City does not provide interpreters. 3. System Reservation Procedure: Groups may request use of the Interpretation System no later than 5 workmg days prior to date of event by contactmg Carmen Atilano at the City of Boulder Office of Human Rights at (303) 441-3140. 4. Use Fees: Secur~ty Deposrt $100.00 Use Fee $25.00 Check m/out $70.00 Late Fee/per day $50 00 Use fee and check in/out fee should be paid for wrth one check A separate check should be written for the securrty deposit. (A fund has been established for assistance to non-profits to cover the $70 check in/check out fee. A non-profit is defined as a 501(c)(3) or a group that has previously received fundmg from the HRC. This fund will be available on a first come first served basis for qualifying groups unt~l all funds are expended.) 5. Security/Damage Deposit Refund: Staff will mspect the system when it is returned. It ~s expected that the system will be returned m the condition m which it was received by the user. Funds will be charged for missing or damaged components of the system at the rates item~zed below. If the amount of damage exceeds the amount of the deposit, the user agrees to pay the difference. Security deposits will be returned to the user by mail approximately three weeks after the event. Replacement cost for system components are. System Pick Up/Drop Off: System must be picked up at the Office of Human Rights, 1101 Arapahoe, 2"d Floor, between 10:00 AM and 4:00 PM the business day before the event and dropped off at the same location by 4:00 pm the busmess day following the event. For each busmess day the equipment is not returned by 4.00 PM, a late fee of $50.00 will be charged. 7. Training: Before usmg the system, a representanve of the usmg agency or organization will need to undergo a brief (20 minute) trainmg with OHR staff on the proper d~stribution, use and storage of the system.
